The new platform claims to offer a complete physician workflow and analytics tool that helps providers track the data that's so important for succeeding in value-based reimbursement.
According to the company, the platform culls data from claims as well as electronic health records to create a detailed picture of a patient's health, giving providers a way to see which patients might be higher-risk without clinical intervention.
The tool also claims to spot gaps in care, as well as areas where costs are high and can be better controlled. It uses hierarchical condition categories used by commercial and government insurers to help physicians coordinate care and spot areas of risk.
