Caradigm launches population health 'team' to help providers take on value-based payment

The company said the growth of accountable care organizations a drove its decision to create the solution.

Population health outfit Caradigm has announced it will be forming what it calls a Care Transformation Team that it claims will help improve the outcomes of population health initiatives for the providers it serves.

The Care Transformation Team, consisting of clinical experts, will give guidance to providers on how to align their specific care delivery needs to Caradigm's capabilities. The team will also help providers pinpoint internal processes that may need to change or expand to optimize for population health.

Caradigm CEO Michael Simpson said in a statement that population health is transforming healthcare, "but how providers adapt and harness this change is an extremely personal undertaking.

"There are multiple paths to population health, such as bundled payments, MSSP, federal programs and clinically integrated networks -- but no two providers, even if they employ the same strategy, will execute it in the same way," he said, adding that the Care Transformation Team's aim is to help providers meet their own unique goals.

The team is led by Caradigm Vice President Vicki Harter, who has over 30 years' experience in the health insurance and healthcare provider markets, focusing particularly on population health strategy. Previously, Harter was the director of Clinical Information Systems at Geisinger Health Plan.

"Population health has created the ultimate nexus between technology and care delivery, but the ability to be successful lies in the details," said Harter.
