MeridianCare partners with Oak Street Health in value-based contract

MeridianCare and Oak Street Health have partnered in a value-based relationship serving Medicare beneficiaries the greater Chicago area.

Oak Street Health operates primary care clinics for Medicare and dual-eligible beneficiaries only through value-based contracts.

Because of this, Oak Street physicians are able to spend twice as much time with patients and the clinics provide minimal wait times, same-day appointments and transportation for eligible patients, MeridianCare said.

The strong patient focus has created a 97 percent patient retention rate and a more than 40 percent reduction in hospitalizations for patients.

Oak Street Health began three years ago in Chicago and has since expanded to 19 locations in the Midwest, according to co-founder and Chief Medical Officer Griffin Myers, MD.

Oak Street and Humana partner in a value-based agreement for Medicare Advantage and dual-eligible patients.

MeridianCare is the Medicare line of business within Meridian, a family-owned and operated company offering health plans in Illinois, Michigan and Ohio.

The company also offers MeridianHealth, serving Medicaid beneficiaries.

The name of the company recently changed from Meridian Health Plan to simply Meridian.
