See which hospitals earned an 'F' from Leapfrog

As 1,000 patients a day still die from preventable errors, Leapfrog report shows which facilities are failing to improve.

SUNY Downstate Medical Center in Brooklyn earned an 'F' in patient safety from Leapfrog.

The Leapfrog Group on Wednesday released its spring Hospital Safety Grade, a biannual program that grades U.S. hospitals on how well they prevent errors, accidents and infections.

Leapfrog rated 2,639 hospitals, and 823 hospitals earned an A, 706 earned a B, 933 earned a C, and 167 earned a D.
