Aurora Health Care, Humana partner on accountable care

The agreement aims to give Humana Medicare Advantage members better care and lower costs at Aurora’s health network.

Humana and Aurora Health Care said they have entered into a new accountable care agreement for Medicare Advantage members throughout the eastern Wisconsin and northern Illinois region.

“As a company, we’re shifting our focus to increase preventative services and ensure efficient, high-quality care to keep members on a path toward lifelong health,” officials said in a statement.

The agreement, effective Jan. 1, aims to give Humana Medicare Advantage members better care and lower costs at Aurora’s 15 hospitals, 159 outpatient facilities, 70 retail pharmacies and 1,500 physicians, according to Humana spokeswoman Cathryn Donaldson.

It centers on Humana’s Accountable Care Continuum and is based on a pay-for-value system promoting integrated care, according to Rick Klein, executive vice president of business development with Aurora Health Care.

Humana members will also  receive customized care through doctor resources, such as real-time “gap in care” identification, predictive modeling and electronic health records.

Louisville, Kentucky-based Humana and Aurora Health Care of Milwaukee, Wisconsin, are also working to shore up their medical records infrastructure and plan to collaborate on disease management and prevention, such as diabetes care and treatment, breast cancer screenings, colorectal cancer screenings and high-risk medication, according to Donaldson.
