The worst-offending hospitals will see their Medicare reimbursements slashed by as much as 3 percent.
The Centers for Medicare and Medicaid Services will penalize 2,592 hospitals in 2016 for having readmission rates that are higher than allowed, the agency reported this week. The penalties will save the federal government $420 million in the fiscal year.

Below is a list of the hospitals that will be fined in 2016, according to an analysis of federal data by Kaiser Health News. The worst-offending hospitals will see their Medicare reimbursements slashed by as much as 3 percent, though many on the list will be penalized less than 1 percent. Many of the hospitals have seen readmissions fall over the past few years, but still received penalties since their rates are higher than acceptable CMS levels. According to Kaiser, all but 209 hospitals on the list were penalized in previous years.
